Care Plans
On entering the home, all residents have a plan of care generated from
a comprehensive assessment which provides the basis for the care to be delivered. The care plan sets out in
detail the action which needs to be taken by the staff to ensure that all aspects of health, personal and
social care of the resident are met.
The residents care plan meets relevant clinical guidelines and includes risk
assessments, paying particular attention to the prevention of falls.
The care plans are reviewed by the qualified named nurse at least once
a month and these are updated in accordance with changing needs and any actions that need to be
taken.
All residents plans of care are drawn up with the involvement of the
resident and agreed and signed by the resident wherever capable and / or representative.
At present Flixton Manor Nursing Home uses a filing system called
“Standex System” which has seen our residents care plans and long term needs assessments become more robust
and more thorough with the outcome being that any significant changes were instantly recognised and acted
upon. This
has proved to be an instant success with the former CSCI inspections and other visiting professionals who
have found them easy to read and simple to reference.
With our focus always looking for ways to improve standards, we have
recently been looking into and have just purchased the latest system that will allow us to have all the
residents care plans, risk assessments, medical information etc, on a computerised system. This software is
specifically designed to meet the administrative needs of the providers of long term care.
This new system will not only make the monitoring and management
of the care delivery more efficient, it will also enable us to reduce the amount of paperwork associated with
other systems, in order to assist us to strive for a paper less environment. The purpose of having all
residents personal information on this system also enables us to confidently hold information in a completely
secure and held in accordance with the Data Protection Act 1998.
