WOUND MANAGEMENT AND PREVENTION
Flixton Manor Nursing Home aims to prevent pressure sores and maintain
healthy, intact skin in all residents wherever possible and to treat pressure sores, effectively if they do
occur.
Every resident is assessed for pressure sore risk on admission using
an approved assessment scale such as the Waterlow Scale. This is re-assessed
weekly or more often if the condition deteriorates.
Residents are encouraged to change position regularly and to keep as
mobile as possible, seeking advice from a physiotherapist if appropriate.
All residents are encouraged to maintain a good nutritional status
which is pivotal in safeguarding healthy skin integrity. All
residents are encouraged to use pressure relieving aids, such as cushions, mattresses etc. Where necessary
residents position changes are carried out in two hourly intervals to ensure pressure relief on high risk
areas.
In the event of a pressure sore developing, or a resident has a
pressure sore on admission to this home then that resident is referred to the Tissue Viability Team, where
advice and a plan of treatment will be commenced. The GP is involved in
the management of pressure sores and advice and medication will be discussed. The Dietician may also
make suggestions for special high protein supplements which promote the healing process.
The choice of treatment for pressure sores depends on the stage of the
ulcer, the presence or absence of any infection and / or necrosis, and the location of the
ulcer. Ulcer dressings are sought and used in accordance with the Tissue Viability Nurses advice
and opinion. The pressure sore is regularly dressed, mapped and graded.
GENERAL
PRACTITIONER
All residents come into this home with their existing GP, where
possible. If a resident comes to us from another area we liaise with our local teams of GP
surgeries, with whom we have good relations with, and provide the resident one of our regular visiting
GP’s.
All our residents’ healthcare needs are overseen by their named GP,
and any medication / changes in condition etc are reviewed on a regular basis by that GP.
If we have any concerns or if we notice any significant changes in a
resident’s condition then the GP surgery is called and a home visit is requested.
We pride ourselves on the good communication that we have with all our
visiting GP’s and have over the years built up some trusting, professional relationships between
ourselves. We value their input, advice, and assistance in order to allow us to promote and provide
quality care for the residents that live in this home.
INFECTION
CONTROL
Infection control is the name given to a wide range of policies,
procedures and techniques intended to prevent the spread of infectious diseases amongst staff, residents and
communities. All the staff working in this home are at risk of spreading infection as they come into
contact with pathogens which can be spread if the staff do not take adequate precautions.
Staff adhere to strict guidelines for infection control and understand
that it is of paramount importance in ensuring the health and safety of the residents as well as
themselves. All protective equipment is provided for the staff who are all trained and regularly
updated on infection control issues and procedures.
Our close link with Trafford’s Infection Control Modern Matron,
enables this home to seek advice and guidance with any infection control issues that arise, or that could
potentially arise and be problematic with the client group we deal with.
This Modern Matron delivers all our training and updates and is in
regular contact via telephone and emails. He is an important
member of our visiting professional network and with whom we have a good working
relationship. He is involved in the care delivery standards at this home and has enabled us to continue
to maintain a high standard of hygiene for our residents.
DIETICIAN
One of our visiting healthcare professionals is from the local clinic
who looks after all our dietary needs of our residents. The dietician is referred to when at any time a resident is
experiencing problems with dietary intake where it may or is having an impact on their well
being.
Our residents are weighed on a monthly basis. However if any
significant loss of weight is recorded then the GP is informed and the weight will be taken
weekly.
If it is apparent that there is a problem with dietary intake, the
dietician is informed and a visit will be planned. The visit will identify
if we need to be offering supplemented drinks to increase calorie intake or the introduction of enriched
foods for residents who only eat small amounts which is having an impact on their health. All changes are
discussed with our cook who is involved with the dietician on a regular basis regarding the resident’s
nutrition. We only enhance our dishes with natural produce such as cream, butter and milk as opposed
to artificial fortifiers which do not offer the same nutritional benefits.
PHYSIOTHERAPY
Here at Flixton Manor Nursing Home we are proud to offer the
regular weekly services of our own physiotherapist as well as access to community based physiotherapy if
appropriate.
Our in house physiotherapy holds a regular exercise class which
incorporates gentle exercises to music that are easily performed sat in a chair for the less
mobile.
The physiotherapist
also performs some gentle heat therapy to relieve pain such as muscular spasm and back
pain. This is also seen as a good opportunity for some one to one time.
CHIROPODY
Our residents can keep their own chiropodist if they have one, or they
can use the services of our visiting private chiropodist for a nominal fee of £10.00.
The visits are conducted privately in the resident's own room if that
is their wish.
The staff at the home remain vigilant with all aspects of a resident's
care and this is no different with regard to foot care.
DENTAL
CARE
Here at Flixton Manor Nursing Home we do have the specialists services
of a PCT visiting dentist and assistant.
Referrals are made directly by the home’s staff and this is passed on
to the dentist who comes into the home to see the specific resident.
The dentist and her assistant will visit private residents as well as
fully funded residents in order to assist us with the maintenance of oral hygiene and care.
Ill-fitting dentures can make life difficult for residents with regard
to discomfort and intake of diet and fluids. Also speech can be
affected, and a loss of some confidence.
Any residents with their own teeth also need regular check ups to
eliminate any problems that may be occurring.
Our visiting dentist also provides our staff with in-house training
with regard to all oral care needs of our residents.
OPTICAL
SERVICES
At Flixton Manor Nursing Home we have the use of a visiting team of
opticians who treat all our residents’ optical needs. This can be anything ranging from eye tests, to choosing new glasses
and frames.
All our residents have this service provided in the comfort of the
home and any new glasses are delivered by the company to us where they are tried and tested by the optician
staff to ensure comfort and suitability.
Some of our residents have conditions of the eyes which means they
require more specialists treatment and go out to the EyeHospital in Manchester.
Again if any of our residents have their own preferred opticians, then
they are more than welcome to remain with that arrangement.
