Personal Care
This Nursing home provides residential and nursing care for adults over sixty years of age, male or female.
Our dedicated and highly experienced team cater to a wide and varied
range of age related conditions and illnesses.
As individual needs change, the appropriate level of care is offered
by providing both residential and nursing care, meaning that the resident is able to remain in familiar
surroundings and with staff who they have built up relations with and who know their individual
histories.
At Flixton Manor Nursing Home we aim to treat each resident as an
individual, and with respect and dignity at all times. Each resident has
a named nurse and an individual care plan. The care plan will cover
their personal and medical history as well as a record of the particular activities they enjoy and any
cultural or religious aspects of their care.
The care that a resident receives will be based on the individual
needs and because of this the assessment process and the care plan for the individual are seen as
crucial.
Needs Assessment
Prior to any new resident entering the home, the qualified staff undertake a full assessment
covering all aspects of needs to ensure that these needs will be met.
New residents that are referred through the Social Services will have also had an assessment
compiled by their social worker which the home receives a copy of.
A thorough assessment plan covers:

Each resident has a plan of care for daily living and longer term
outcomes based on the assessment.
